Start with the code from your clinic
Visit Patient intake and enter your clinic-issued access code and date of birth. You do not need to create a staff or clinic account.
Complete your questionnaire
Work through the sections, review your details, and save your progress as you go. You can resume on the same device while your session is active. If you sign out or your session expires, ask your clinic for a new code.
Review and submit
Check your answers and enter the name of the person signing. Once submitted, the intake is sent to your clinic for review and becomes read-only. Contact your clinic if your information changes.
Your clinic is your point of care
For appointment changes, medical questions, access codes, or questions about your patient record, contact your clinic directly. The general contact form is for platform inquiries and should not include health information.
